This form would have either been provided to you by the person or agency who is requiring you to have this background search done, or you would have been asked to locate the appropriate form on-line. Exceptional Notary does NOT provide these forms.

If you find that you are UNABLE to print your form, or need to purchase an FBI Card (FD-258), Exceptional Notary can provide an FBI Cards for you for an ADDITIONAL $1.50 per card (Will be added to your Fingerprinting Fees)
